Before regional selection trial

Forms to be submitted prior to regional selection trial:

• QRSS Transfer of duty form (pdf, 1454kb)
• QRSS Staff Consent form (pdf, 816​kb)​​​
  
• Declaration of conflict of interest​​​
  
• ​​contact the MNSS office to obtain this form
• ​Relevant ​Risk Assessments and risk management documents​
  

Forms to be submitted post regional selection trial:

• Regional team list (manager's Excel file)
• Project consent and third party consent summary (manager's Excel file)
• to be submitted with team list
  
• Playing apparel order (manager's Excel file)
  
• Team member training schedule (DOCX, 41KB)
  
• to be submitted to RSSO for approval. If you can send in the information prior to the selection trial, the information can be noted on the team invitation/letter.
  
• Team training letter (DOCX, 40KB)
  
• to be submitted to all team members once approved by the RSSO if not noted on the team invitation/letter.​​
  

After state championship

Forms to be submitted post Queensland School Sport state championship:

• Officials report (manager's Excel file)
  
• Team coach report (DOCX, 38KB)
  
• Official's claim for allowances must be submitted no later than TWO WEEKS post your respective State Championship.​
  
• further information will be provided by the MNSS office closer to your travel day.​​
